How the market splits

Longevity programs in 2026 cluster into three archetypes. Diagnostic-anchored programs (Human Longevity Inc., Fountain Life) front-load whole-body imaging, deep bloods, and genomics — the annual fee buys certainty. Concierge-medical programs (Extension Health, HUM2N) spread cost across quarterly physician time, protocols, and in-clinic therapies. Residential retreats (Clinique La Prairie, Chenot, SHA, Lanserhof) charge per-stay and are typically bought in addition to, not instead of, one of the first two.

Where clinics diverge most is what happens between visits. Diagnostic programs assume you have a GP elsewhere; concierge programs are the GP. That distinction — not the headline price — is where most buyers over- or under-pay.

What you're actually paying for

  • Imaging depth. A whole-body MRI + coronary CTA + brain MRI stack accounts for roughly $6–9k of the diagnostic-anchored fee. Programs without it are $10k+ cheaper for a reason.
  • Physician time. Concierge programs cost more per hour of clinician contact than any private GP tier — that's the product.
  • Data continuity. The programs that hold up over 3+ years are the ones with a single record following you across scans, bloods, wearables, and interventions. Most don't.
  • Brand and setting. Swiss residential programs charge a location premium of 30–60% versus clinically comparable protocols elsewhere in Europe. That's a real cost, not a hidden one.

Where Portugal fits

Portugal is currently under-supplied at the concierge tier — Lisbon has two credible entrants and the Algarve has one, all under 18 months old. Pricing sits 20–35% below London equivalents for a comparable protocol.
